Haywood Family Eye Care complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. We do not exclude people or treat them differently because of any of these.

Our commitment

Every patient who walks through our door deserves the same care, the same respect, and the same access to information about their eye health. That is the commitment we make under federal law and the one we choose every day in our office.

Help with communication

We want every patient to understand their care. If a disability or your preferred language makes communication harder, we provide help — at no cost to you.

If you have a disability

We offer free aids and services to help you communicate with us, such as:

  • Qualified sign language interpreters.
  • Written information in other formats — large print, audio, accessible electronic formats.
  • A staff member to read forms aloud or help you fill them out.
  • Patient walking assistance from the parking lot to the exam room.

If your preferred language is not English

We offer free language assistance, including:

  • Qualified interpreters by phone or in person.
  • Written information in other languages on request.
  • Appointments with Dr. Przepiora, who sees patients in Polish, Spanish, and English.

If you believe Haywood Family Eye Care has failed to provide these services or has discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with our Civil Rights Coordinator — or directly with the federal government, or both. We will not retaliate against you for filing a grievance.
